Top 20 Entry Form & Payment

Entries will be mailed around DATE, and verified emails will also receive the necessary instructions via email at that time as well.

Entries will close at midnight on NEED DATE

By that time all payments and first drafts of materials are due.

Invitational Checklist

• Entry Fee $100

• Catalog Advertisement Fact Sheet

• Photo for Catalog

• Three Generation Pedigree (use template emailed)

• Music selection to play during invitees individual, on the event night (limit selection to 2 minutes)

• Request for (#) additional Rosettes ($50 each) or (#) medallions ($25 each)
